If I’ve setup a wildcard subdomain, does that mean any website can point to it with a CNAME, or do I need to still add those websites in as a custom domain?

If you’ve setup *.mydomain.com and have it verified and certificate issued, then anything-here.mydomain.com will reach that service.
If you then mean then providing something.mydomain.com as a CNAME to another domain for like a SaaS-like setup, e.g. as a CNAME on something.customerdomain.com, then no, that wouldn’t reach the service as Render wouldn’t be aware of something.customerdomain.com. It would need to be added as a custom domain to the Render service.
